The cover sheet serves as a balance sheet the accounting. If the account balances the total of the numbers in the top half for the cover sheet will balance with the total of numbers in the bottom half of the cover sheet. More information on the sections of the cover sheet is reviewed below. Although the sample accounting is simple, review of this sample along with the instructions will provide you with a a good understanding of the information and form for conservator accountings.The accountings (to which the cover sheet is to be attached) can be prepared as an Excel spread sheet, in a word processing program or other form that provides the required information and is legible, subject to approval of the Commissioner.

Reviewing the Instructions and sample accounting will provide most, if not all, the information you need to prepare a property accounting. The .pdf forms can be completed via computer online and printed on your local printer for signing and filing. The parts of the cover sheet are listed below. An itemization of the entries for each category must be stated in the accounting and with supporting documents for the entries. The total numbers on lines 5 and 10 must balance. 1. Beginning Assets (from Parts 1, 2, 3 & 4 of the Trust Inventory or ending assets of a prior account). 2. Receipts 3. Gains on Asset Sales 4. Adjustments 5. Total of 1, 2, 3 & 4 (must equal total in line 10). 6. Disbursements 7. Losses on Asset Sales 8. Distributions 9. Assets on hand (remaining) at the end of the accounting period). 10. Total of items 6, 7, 8, 9 (total must equal the total of Line 5).
Beneath line 10, there is a line to state amount total government benefits received by the conservator if they are appointed as designated representative for the incapacitated person and receiving such benefits. If the conservator is not a designated representative then they must itemize the receipt and expenditure of such funds in their accounting. The Conservator's final accounting will show zero assets on hand as all assets have been disbursed or distributed to the executor or administrator of the incapacitated person's estate, if they have died, to the incapaciated person, if a circuit court determins that he or she is no longer incapacitated, or in accordance with an order of the Court specifying property distribution of the remaining assets. Filing Fees The total filing fee pay payable with the filing an accounting includes the Commissioner's filing fee as provided in the Uniform Fee Schedule Guidelines for Commissioners of Accounts and the filing fee of the Clerk of Court.
