Conservator - Inventory and Instructions
Links are provided below to the form and instructions for a Conservator's Inventory:

The Inventory form and instructions sets forth four categories of property of the incapacitated person to be listed on the Inventory: (i) the personal estate (personal property) under the conservator's supervision and control, (ii) real estate, (iii) the legal or equitable ownership interest in any real or personal property that will pass to another at the incapacitated person's death by a means other than testate or intestate succession, and (iv) any periodic payments of money to which the incapacitated person is entitled (as examples, social security and/or pension payments). The Court forms and instructions may add additional sub-categories that may apply based on circumstances. Follow the instructions.

Tip. The Inventory form at the link above is located at the Virginia Judicial System website. The .pdf file is a fillable form that you can complete on the site and the form will add the totals, and when completed can be printed on your local printer for for signing and filing with the Commissioner.

Additional Form Information

  • The values to be stated for assets listed in the inventory are to be based on fair market value as of the date of the conservator's qualification, and not the date the inventory is prepared.
  • Note that similar property, such as personal effects and household property may be grouped together with a total value; however, if any article is worth over $500 it should be listed separately.

  • For checking, savings and other accounts, Commissioners will typically accept the last four digits of the account number for security purposes.

  • The Conservator should inform the Commissioner if they are appointed as representative payee for social security, SSI or other government based benefits, and provide a copy of the letter they received from the agency confirming such appointment as this may affect whether disbursement or distribution of such source of income must be reported in detail on accountings.
