The Inventory form and instructions sets forth four categories of property of the incapacitated person to be listed on the Inventory: personal property, real estate over which the guardian will have power of sale, other real estate in Virginia, the minor's non-Virginia real estate, the minor’s legal or equitable ownership interest in any real or personal property that will pass to another at the minor’s death by means other than testate or intestate succession, the minor’s rights to periodic payments from any agency of the U. S. government, the minor’s rights to periodic payments from any other source.

The .pdf file is a fillable form that you can complete on the site and the form will add the totals, and when completed can be printed on your local printer for for signing and filing with the Commissioner.

Additional Form Information

• The values to be stated for assets listed in the inventory are to be based on fair market value as of the date of the conservator's qualification, and not the date the inventory is prepared.

• Note that similar property, such as personal effects and household property may be grouped together with a total value; however, if any article is worth over $500 it should be listed separately. 
  
  
• For checking, savings and other accounts, Commissioners will typically accept the last four digits of the account number for security purposes. 
  
  
• The guardian should inform the Commissioner if they are appointed as representative payee for social security, SSI or other government based benefits, and provide a copy of the letter they received from the agency confirming such appointment as this may affect whether disbursement or distribution of such source of income must be reported in detail on accountings. 

Filing Fees

Filling fees typically include the fee payable to the Commissioner plus the filing fee that is payable to the Clerk's of Court. Guidelines for Commissioners' fees are provided in a letter section below. Clerk's fees may vary and increase based on the number of pages.
